- A fully completed and signed application form. If the applicant is under 18 years of age, the form must also be signed by both parents or legal guardians.
- Original passport and a copy of personal passport information page, immigration stamp page, and 2 empty pages
- Two recent passport-sized photographs (3.5cm x 4.5cm) taken within the last 6 months
- National ID Card and Family Card, or a copy of your residence permit with validity of minimum 6 months
- Identity Proof Documents (the latest should be three months old) like Employment Certificate, Birth Certificate, etc
- A copy of your valid residence permit in Indonesia (KITAS/KIMS/KITAP). This permit must be valid for at least three months after your intended departure from the destination country.
