A fully completed and signed application form. If the applicant is under 18 years of age, the form must also be signed by both parents or legal guardians.

Original passport and a copy of personal passport information page, immigration stamp page, and 2 empty pages

Two recent passport-sized photographs (3.5cm x 4.5cm) taken within the last 6 months

National ID Card and Family Card, or a copy of your residence permit with validity of minimum 6 months

Identity Proof Documents (the latest should be three months old) like Employment Certificate, Birth Certificate, etc

A copy of your valid residence permit in Indonesia (KITAS/KIMS/KITAP). This permit must be valid for at least three months after your intended departure from the destination country.

Bank mutation details for the last three months (original transaction details issued with the stamp by the bank mention account name, number, and bank name) (Each page Certified by Ministry of Law and Human Rights (Apostille)
