  • Completed and signed application form (signed by both parents for applicants under 18).
  • Original passport and one copy of the personal information page and immigration stamps.
  • Recent passport-sized photograph (4.5 cm x 3.5 cm, white background, matt/semi-matt finish, taken within 3 months; long hair neatly tied).
  • Copy of valid Indonesian residence permit (KITAS/KIMS/KITAP, minimum 6 months validity) OR valid Indonesian Visa/VOA (minimum 2 weeks validity) with full Indonesian address.
  • Complete address in the applicant’s country of origin.
  • Proof of Identity (documents issued within the last three months):
    • Employed: Original employment certificate (draft provided).
    • Student: Certificate of Enrollment.
    • Minor: Birth certificate and Letter of Consent.
    • Entrepreneur: Business license certificate (SIUP/NIB).
  • Letter of Introduction (LOI) from a local contact in Singapore (if applicable, mandatory for business trips). The local contact must be a Singaporean citizen (SC) or permanent resident (PR) aged 21 or older, or a Singapore-registered company.
  • Supporting documents, if applicable.
